I present to you now, a case-based customer implementation that highlights hospital process improvements leading to significant time-savings, and consistent delivery and documentation of care:
Process before Concurrent Insight
This particular hospital utilizes two employees, Abstractors, to scour the floor each day with a set of clinical guidelines. The Abstractors are expected to review each of the cases each day, evaluate changes in care, and review each of the new inpatients based on the provided guidelines. The next step in this process is to provide the relevant data (based on the guidelines and their interpretation) to the manager of Core Measure and Quality data. The team will review the data and determine which patients to focus on. The aforementioned process takes 4-5 hours to complete the daily review. Once the review is complete, it doesn’t leave much time for additional analysis or corrective action.
Improvements with Concurrent Insight
CI will have two primary users: the Abstractors, along with the management team. The Quality manager wants to turn the Abstractors from data "hunters" to data "analysts". Instead of spending the majority of the time pulling together data, they now will have time where they can also act on data.
The focus will shift to:
- Identifying gaps in care
- Following up with the nursing staff as needed
- Ensuring that the documentation is filled out in a timely fashion.
The expected short term benefit is an additional 40-50 hours per week of time for the abstraction team to analyze and act on data instead of simply only gathering the data. The long term impact will be more consistent delivery and documentation of care, which will lead to better quality patient care and improved performance in the quality measured space.
