Things to do before attestation

It is the home stretch for 2014 so make sure you cover everything that you need to do before you attest::

  1. Prepare your audit documentation, including:
    1. All relevant Standard Operating Procedures and Policies
    2. System configuration screenshots
    3. System “up-time” documentation (logs, screenshots, etc.)
    4. Documentation that guided decisions made about measure implementations and reporting (Final Rule text, CMS FAQs, Vendor FAQs)
    5. All billing for CEHRTs, meeting minutes, and email trails that are relevant
    6. Vendor letters to show population method calculations, possession letters, etc.
    7. Documentation to support any decision to roll back to an earlier CEHRT Edition or Stage in FY2014
    8. Documentation to support all Yes/No measures (public health registries, etc.)
    9. Clinical Decision Support rules and supporting documentation
    10. All training materials and supporting material for the measures being attested to
    11. Security Risk Analysis
    12. If it is not stored in your EHR, all documentation to show which patients accessed portal and which encounter data was made available to the portal
    13. If not stored in your EHR, documentation showing which transition summaries were received by providers
  2. Store all audit documentation in place where it can be found if an audit happens, regardless of staff turnovers and computer failures
  3. Go to the CHPL website (http://oncchpl.force.com/ehrcert?q=chpl) and get a certification number for your CEHRTs. If you are using a 2011 certified system or a mix of 2011/2014 be sure to select the correct versions of the certified software so that the CHPL website will generate a certification ID that gives you the correct options on the attestation website. Be sure to save all documentation related to the CEHRT ID.
